Dignity Kare Corporation provides care coordination and support services to Medi-Cal members across California through CalAIM, the state's initiative to deliver whole-person care. We help individuals and families navigate complex health and social needs — connecting them to the right services, at no cost to eligible members.

Dignity Kare supports adults across California who face barriers to coordinated care, including:
Adults experiencing homelessness
Adults at risk of avoidable emergency room visits or hospitalization
Adults at risk of long-term care institutionalization
Nursing facility residents transitioning back to the community
Adults with intellectual or developmental disabilities
